The services being delivered, and the staff providing these, should be organised and co-ordinated to provide seamless care across the Mid Yorkshire system.
For true integration to occur health, social care, voluntary and third sector services and providers need to work together across organisational and professional boundaries and step outside of the ‘traditional’ roles and functions they may have previously performed.

Workforce Planning
Population based workforce planning is a proactive planning approach that focuses on addressing the specific needs of a defined local population.
The primary care home model involves reviewing the population of an area and creating smaller local groups of citizens with similar characteristics and needs, allowing workforce planning to be undertaken that is personalised to each area.
Services can be developed to fit around local people, by considering various aspects of health, social care and wellbeing and developing specific strategies to address these. The service user perspective should be taken throughout planning to ensure the needs of citizens are being met and care remains person centred.
