

Wakefield District HCP:
Wakefield District HCP works to improve the health and wellbeing of local people, by reducing health inequalities, providing continuity of care and improving our services. Our partnership includes NHS organisations, Wakefield Council, Healthwatch Wakefield, housing, voluntary and community sector organisations.
To achieve this vision, the workforce must share best practice and integrate services to ensure care is co-ordinated and person centered.
This website has been developed for member organisations to share and contribute to learning and development resources across the Wakefield District HCP. This will allow standardised training delivery and practice alongside shared workforce development. In this way the health, social care and wellbeing workforce can truly ‘work together, learn together and grow together’.

Wakefield District HCP is a partnership of health, social care, housing, private and voluntary organisations from across Wakefield, which aims to drive integration of services to deliver person-centered care.
For true integration to occur, healthcare, social care, voluntary and third sector services and providers need to work together across organisational boundaries to deliver seamless care.
By sharing training resources across the Wakefield District HCP, we can ensure consistency of training and avoid duplication, saving time, effort and resources.
There is a growing and changing demand for how care is provided, with a shift in thinking away from traditional role boundaries towards new.
To ensure we are delivering high quality, dependable and valuable care services, it is important we support the wellbeing of our workforce.
The Wakefield District HCP system need a robust, agile and flexible cohort of leaders with a common set of principles and standards.
To deliver hgih quality care, we need to attract, recruit, retain and develop an excellent health, care and support workforce.
